What this is: A running record of every defect found, its severity, and what happened to it. Logging every defect turns individual problems into a checkable defect rate and reveals root causes that keep repeating.

Sticky Note Tip
Log every defect, including minor ones accepted with a concession. If you do not count every defect, you cannot see a pattern forming. Rejected items feed directly into Volume 13's waste rate.
